I-9 Form  
INS has changed List A of Acceptable Documents on the back of the I-9. Do not use the following to establish identity and employment eligibility: Certificate of U. S. Citizenship,(INS Form N-560 or N-561), Certificate of Naturalization (INS Form N-550 or N-570), INS form I-151, Unexpired Reentry Permit (INS Form I-327), Unexpired Refugee Travel Document (INS Form I-571). Do not accept any of the documents listed to complete the Form I-9. Since an updated form will not be published until late 2002 or early 2003, a revised sample Form I-9 has been posted below. On List A, the items highlighted in yellow are acceptable, while the ones with a red strike through are unacceptable.

According to the Immigration Reform and Control Act of 1986, all employers are required to verify an employee’s identity and eligibility to work in the United States if hired after November 6, 1986. The Immigration and Naturalization Service Form I-9, also known as the "Employment Eligibility Verification Form'' is used to document the employee’s identity.
